Eligibility : Transitions Take Your Shot Instant Win Promotion (the “Instant Win Game”) is open only to legal residents of the fifty (50) United States and the District of Columbia who are at least eighteen (18) years old at the time of entry. Void where prohibited.

How To Enter : To enter without purchasing Transitions Lenses, visit www.transitions.com/takeyourshot and click on the “Don’t have a COA or LIN Code?” link and follow the links and instructions to have a COA Code and LIN Code emailed to you. Once you have the Codes, visit the Website and complete and submit the online entry form including a valid home address (P.O. Boxes are not permitted). After submitting the form, complete a short survey and follow the links and instructions to proceed to the Instant Win Game and play the Instant Win Game. You will be able to play one (1) time for each unique COA Membership Number and Location Identification Number you enter.

Limit : Each participant may play three (3) times during the Promotion Period by any method or combination of methods of entry.
