We have used the puppy pads for some time. We were sending the proof of purchase in for the rebate. We need to obtain the form to submit the $5.00 rebate claim. How can I obtain a form for this purpose? Is this rebate still being honored?
I tried to mail my 5.00 rebate form in with the 10 proof of purchases of packs, december.21.2009,before deadline 12-31-09.and i got it back today 12-31-09.it said address unknown.how am i to get my rebate back for the ten packs .
I also mailed my proofs of purchase for the $5.00 rebate well before the expiration deadline and got it back today saying it can’t be delivered to the address shown. How do I claim this rebate?
