Enfamil Smart Choice Sweepstakes

Eligibility :
Enfamil^®; Smart Choice™ Sweepstakes (the “Promotion”) is open to parent(s) who are at least eighteen (18) years old and are legal residents (as of date of entry) of the fifty (50) United States or the District of Columbia. To be eligible, parents or expectant parents must have a baby or babies who are legal residents of the fifty (50) United States or the District of Columbia as of the date of birth, and who are or will be born between January 1, 2007, and July 1, 2009 (inclusive) (“Eligible Babies”).”

Timing :
The Promotion begins on April 15, 2008 at 12:00 a.m. Eastern Time (“ET”) and ends on October 15, 2008 at 11:59 p.m. ET (the “Promotion Period”).

How to Enter :

a. To Play the Instant Win Game: During the Promotion Period, go to http://www.enfaville.com and fill out the online entry form. Complete and submit the online entry form including a valid home address. P.O. Boxes are not permitted. Once you submit the form, you will be directed to a virtual village. Follow the directions to explore the village and find the location of the day’s Instant Win Game, which will vary throughout the Promotion Period. The Instant Win Game results will be instantly displayed. Potential winners of the Instant Win Game will be provided directions for validation. All potential winners are subject to verification before any prize will be awarded. Administrator is responsible for the functionality of the Instant Win Game. Limit: Participant may play one (1) time per day during the Promotion Period.

b. To Enter the Sweepstakes: There are two (2) ways you can enter the Sweepstakes during the Promotion Period:

Online Entry via Code: To enter online, during the Promotion Period, request either a specially-marked Enfamil^® diaper bag, a first trimester gift kit (gift kit determined by Sponsor in its sole discretion), an Enfamil LIPIL^® changing pad, a breastfeeding resource kit, a breastfeeding support kit or Enfamil LIPIL^® formula feeding kit (determined by Sponsor in its sole discretion) from a participating doctor’s office or hospital, while supplies last (each a “Product”). Each Product will contain a game-piece with a code (the “Code”). You may also obtain a Code through other methods as may be added from time to time, in Sponsor’s sole discretion. During the Promotion Period, visit http://www.enfaville.com, and follow the links and instructions to complete the registration. Complete and submit the registration including a valid home address. P.O. Boxes are not permitted. Then locate the hospital in the virtual village and enter the Code. Upon entering the Code, you automatically will receive one (1) entry into the Sweepstakes.

Offline Entry via Mail: To enter without obtaining a Code, during the Promotion Period, hand print your name, address, day and evening phone numbers, email address (if any), your age, and the birth date or expectant birth date of your baby on a 3″ x 5″ piece of paper and mail it with proper postage to: “Enfamil^® Smart Choice Sweepstakes,” c/o ePrize, LLC, P.O. Box 1746, Grand Rapids, MI, 49501-1746. You will receive one (1) entry into the Sweepstakes. Limit: One (1) entry per envelope. All mail-in entries must be handwritten, and to receive an entry into any Monthly Entry Period, postmarked and received by the dates specified in the table in Section 3. All entries become the exclusive property of Sponsor, and none will be acknowledged or returned. Proof of sending or submission will not be deemed to be proof of receipt by Sponsor. Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, illegible, misdirected or postage-due entries, which will be disqualified.

Limit :
Each participant may enter up to ten (10) times during the Promotion Period, regardless of method of entry.

Sweepstakes Prizes: FOUR (4) GRAND PRIZES: A $27,957.86 fixed deferred annuity (with a guaranteed account value of $50,000 after eighteen (18) years, subject to all terms and conditions of the annuity contract, including surrender penalties; assuming interest rates applicable as of January, 2008; interest rates subject to market fluctuations) that winner can apply to his/her child’s educational expenses. Approximate Retail Value (“ARV”): $27,957.86. FIFTY (50) FIRST PRIZES: A $1,000 Babies”R”Us^® gift card. Terms and conditions apply. ARV: $1,000. Odds of winning a Sweepstakes Prize depend on the number of eligible entries received during each Entry Period.

Instant Win Game Prizes: ONE THOUSAND (1,000) SECOND PRIZES: A Fisher Price “Laugh and Learn” Toy of Sponsor’s choice. ARV: $50. TWO THOUSAND (2,000) THIRD PRIZES: An Enfamil^® travel pack consisting of two (2) 6-packs of ready-to-go formula. ARV: $20.00. ONE THOUSAND (1,000) FOURTH PRIZES: A Baby Einstein DVD. ARV: $19.99. Odds of winning an Instant Win Game Prize depend on the number of eligible prizes available divided by the number of Instant Win Game plays. Limit: One (1) Instant Win Game Prize and one (1) Sweepstakes prize per person.
