OR 2.) On an Official Sweepstakes Entry Form or 3″ x 5″ piece of paper, hand-print your name, complete address, and daytime, date of birth and evening telephone number, and mail to: LU Biscuits Fashion Experience Entry, P.O. Box 4805, Oak Brook, IL 60522-4805. Entries must be received by July 28, 2008.
